Conard Parent Questionnaire
The questions below are designed so that your child’s counselor can write a more comprehensive letter of recommendation. The more thorough your responses, the more helpful they will be. Feel free to handwrite your responses, but we would prefer to have them typed. The questionnaire is available at www.whps.org/conard and can be sent electronically or to Conard High School, 110 Beechwood Road, West Hartford, CT 06107, in care of the school counselor.

1. Choose three or four adjectives that describe your child and give one anecdote to support each adjective. Example: Tenacious – never started on the little league team, but played baseball for eight years, always going to practice even though he had only one hit in all the games he played.
2. In what areas has your child shown the most growth and development during their high school years?
3. What do you consider the most outstanding accomplishments of your child during their high school years? Why did you select these as important?
4. Are there any colleges to which you will encourage your child to apply? Is there a particular college setting that you feel your child will do best in, such as location, size and cost? If so, which ones and why?
5. Are there any unusual or personal circumstances that have affected your child’s educational or personal experiences?
6. Is there anything we have spoken about in the past that you would like to remind me of?
