All students are required by law to have a physical examination done by a Connecticut physician on the blue State of Connecticut health form before admission to school. Please submit this form to the school nurse.
Students transferring from within Connecticut may enter with their physical on the state form from another in state school. Out of state students may enter if their previous physical was done with the past year and complies with state requirements. Those students must see a Connecticut physician within a year of admission. Students entering from another country must have a Connecticut physician do the physical prior to admission and must have a PPD if coming from a high risk country.
A physical exam is also required in Grades 3, 6 and 10.
A physical examination includes hematocrit or hemoglobin tests, height,
weight and blood pressure; an updating of immunizations as recommended;
vision, hearing, speech and gross dental screenings.
