REGISTRATION DOCUMENTATION FOR SCHOOL:

All students born in the United States must present an original full-size birth. (wallet size is not acceptable)


 


If the student is not born in the United States we require a passport or green card. (you may choose one) 

                            or                 


If a child resides with a legal guardian other than the parent. The legal guardian must provide:

• Valid Photo ID.
• Child’s FULL-SIZE birth certificate (original copy)
• State of Connecticut issued documentation that they are the student’s legal guardian. (Notarized letter from the biological parent is NOT  acceptable).

RESIDENCY DOCUMENTATION FOR WEST HARTFORD
Please provide documentation from one of the following that applies:

 




HOMEOWNERS:

(CHOOSE ONE OF THE FOLLOWING and a CURRENT UTILITY BILL)

• Current mortgage statement
• Homeowner’s property tax statement
• Homeowner’s insurance policy

         
RENTERS:

• Current rental agreement (original copy)
• and Current UTILITY BILL

  
IF DWELLING WITH ANOTHER WEST HARTFORD RESIDENT:

If the parent or legal guardian is not a West Hartford resident but is staying with a West Hartford resident, then both parties must sign a Certification of Residency and both parties must provide:

•  Valid Photo ID
• West Hartford resident must provide their proof of residency.
